NASA has requested an independent review of the investigation into the unexpected loss of material from the Orion spacecraft's heat shield during a test flight in 2022. The heat shield, made of a material called Avcoat, experienced significant cracking and chipping during reentry. Engineers are still investigating the root cause of the issue, as it poses risks to future missions.
